Does Testosterone Therapy Affect Fertility? What Men Need to Know

Many men are surprised to learn that testosterone replacement therapy (TRT) can lower sperm production.

If a patient plans to have children, this conversation needs to happen before starting therapy.


Why TRT Impacts Fertility

When a man takes external testosterone, the body reduces its own natural production. This can suppress signals from the brain (LH and FSH), which are necessary for sperm production.


The result:

  • Reduced sperm count

  • Possible infertility while on therapy

  • Testicular shrinkage in some cases


For some men, sperm production returns after stopping TRT — but not always immediately.


Who Should Be Cautious?

Men who:

  • Are actively trying to conceive

  • Plan to have children in the future

  • Have borderline fertility already


These patients need a different approach.


Are There Alternatives?

Depending on the situation, options may include:

  • Clomiphene

  • hCG

  • Monitoring without TRT


Treatment should match the patient’s goals — not just their testosterone number.

If preserving fertility matters, it should be part of the treatment plan from day one.
